Cumulative minority stress and suicide risk among LGBTQ youth.

Exposure to minority stress is the primary mechanism through which lesbian, gay, bisexual, transgender, queer, and questioning (LGBTQ) youth experience a greater risk for suicide. The current study examines the association of LGBTQ-based cumulative minority stress with suicide risk using online survey data collected from 39,126 LGBTQ youth ages 13-24 in the United States. Youth who reported four types of minority stress had nearly 12 times greater odds of attempting suicide compared to those who reported none. Transgender and nonbinary youth and American Indian/Alaskan Native youth had higher odds of reporting three or more minority stress experiences. The strong association of cumulative risk with attempted suicide and disproportionate exposure among marginalized members of the LGBTQ community highlight the need for suicide prevention to prioritize those at greatest risk and for research examining LGBTQ suicide risk to employ cumulative risk models.

Trends in Deaths by Suicide 2014-2019 Among Lesbian, Gay, Bisexual, Transgender, Queer, Questioning, and Other Gender/Sexual Minority (LGBTQ+) Youth.

This study describes changes in proportions of lesbian, gay, bisexual, transgender, and other gender/sexual minorities (LGBTQ+) among total deaths by suicide in American 11-29 year olds between 2014 and 2019. Data came from the Centers for Disease Control and Prevention's (CDC) National Violent Death Reporting System (NVDRS). All cases classified as deaths by suicide since 2014 that had valid data for sexual orientation or transgender identity were included from states that started participating in NVDRS before 2016. The latest year of data available was 2019. Valid n = 4,086, including 673 LGBTQ+ cases. Analyses used "reverse regression" with gender/sexual orientation as the dependent variable, essentially estimating contributions of timing of fatal self-injury and other variables to relative risk of a case being LGBTQ+. The proportion of decedents who were LGBTQ+ increased between 2014 and 2019. Post-hoc analyses suggested an inflection point or break in the trend at November 9, 2016. The proportion of decedents who were LGBTQ+ increased from 13% before to 20% after that date, driven largely by increases from 2.0% to 6.5% among transgender youth and from 0.5% to 1.7% among those described in medical examiner and law enforcement reports as "struggling" or questioning. Asian bisexual females and gay males were especially overrepresented. Previous research found that the policy environment affects LGBTQ+ youths' mental health; this study generalizes those findings to actual deaths by suicide. LGBTQ+ youth at risk should be assessed for anxiety, trauma, and perceived physical threat from the political and rhetorical environment.

Methods of attempted suicide and risk factors in LGBTQ+ youth.

This study aimed to identify patterns of attempted suicide methods and risk factors in lesbian, gay, bisexual, transgender, queer and other gender/sexual minority (LGBTQ+) youth by comparing them to non-LGBTQ+ youth. A total of 9281 Chinese college students completed the study, 8313 participants were included in the analysis of which 1032 were LGBTQ+ youth and 7281 were non-LGBTQ+ youth. Sociodemographic information was collected along with several scales and self-report items. This included anxiety scores using the Generalized Anxiety Disorder (GAD)-7, depression scores using the Patient Health Questionnaire (PHQ)-9, mania score using the Altman Self-Rating Mania (ASRM), Childhood Trauma Questionnaire (CTQ), and Intimate Partner Violence (IPV) victimization using the World Health Organization's definition. Self-report items were used to further collect history of non-fatal self-injurious behaviors, parents' related risk factors, history of suicide attempts, and methods of attempted suicide. The prevalence of attempted suicide in LGBTQ+ youth was 4.2%, which in comparison was more than four times higher than non-LGBTQ+ youth. LGBTQ+ youth 19-22 years old were at a significantly higher risk of attempting suicide than non-LGBTQ+ youth. The most common method of attempted suicide was cutting of the wrist, followed by jumping from a high height. There were some common suicide risk factors that were similar between LGBTQ+ and non-LGBTQ+ youth, such as living in a rural area, having a history of a psychiatric disorder, and having a history of non-fatal self-injurious behaviors. The associated increased risk factors for LGBTQ+ youth were having a higher score for CTQ-emotional abuse and CTQ-sexual abuse. The causality of risk factors to attempted suicide cannot be assumed due to the cross-sectional nature of the survey. Also, due to the relatively small sample size within the subgroups, we did not assess LGBTQ+ youth separately according to the different sexuality groups. LGBTQ+ youth showed a higher prevalence of attempted suicide when compared to their heterosexual peers. Emotional and sexual abuse showed higher rates among LGBTQ+ youth, the cumulative effects of childhood trauma might explain the difficulties in developing healthy coping styles.
