The Role of Survivorship Care for Patients with Glioma.

To discuss the role of survivorship care and survivorship care plans, including a recently developed neuro-oncology-specific plan care (https://www.soc-neuro-onc.org/SNO/Resources/Survivorship_Care_Plan.aspx) for adult patients diagnosed with primary glial neoplasms and the necessary educational needs of oncology nurses. Published peer-reviewed literature and resources from cancer and neuro-oncology professional organizations and patient advocacy organizations. The current Commission on Cancer mandates adult cancer patients treated with curative intent each receive an individualized survivorship care plan. Patients with glioma are likely to benefit from receiving survivorship care, including survivorship care plans aimed at addressing the complex and evolving needs of this unique patient population throughout their illness trajectory. Nurse professionals are critical to the development and implementation of cancer survivorship care. This growing leadership role presents oncology nurses with specific and new educational needs regarding survivorship care.

To review advanced practice nursing roles in planning, implementing, and evaluating survivorship care. Review of the literature, published articles, government and organizational reports. The increased focus on improving post-treatment cancer care presents opportunities for advanced practice nurses to meet the physical and psychosocial needs of cancer survivors. As experts in the comprehensive delivery of care, oncology advanced practice nurses are positioned to initiate, deliver, and evaluate survivorship care through innovative models.

Survivorship care planning: one size does not fit all.

To describe the delivery of survivorship care and methods to stratify risk to support the notion that "one size does not fit all." Published articles between 2007 and 2014 and original research findings. The development and implementation of survivorship care into practice provides barriers and opportunities. National mandates are pushing the delivery of a survivorship care plan, which requires the ability to develop and deliver this tool and the necessary health care delivery model to manage the unique needs of each cancer survivor. Oncology nurses and advanced practice nurses will play a crucial role in the development of survivorship care from education and assessment to the delivery of coordinated care.
