The challenges of the integration of cancer survivorship care plans with electronic medical records.

To provide an overview of issues and challenges associated with integrating development of survivorship care plan processes with electronic medical records (EMRs). Published peer-reviewed literature. Evidence seems to indicate that survivorship care plans have value to survivors, oncology specialist providers, and primary care providers. Yet, the existence of cost and time restraints are major barriers to creation and use of survivorship care plans, and the expectations that EMR can simplify and expedite survivorship care plan development have yet to be realized. Nurses participating in the development of survivorship programs can contribute to successful implementation of EMR-facilitated survivorship care plans by involvement in strategic planning processes, and establishment of reasonable timelines to address the known and unknown barriers, and assuring the resulting EMR product includes essential data and information.

Survivorship care planning: one size does not fit all.

To describe the delivery of survivorship care and methods to stratify risk to support the notion that "one size does not fit all." Published articles between 2007 and 2014 and original research findings. The development and implementation of survivorship care into practice provides barriers and opportunities. National mandates are pushing the delivery of a survivorship care plan, which requires the ability to develop and deliver this tool and the necessary health care delivery model to manage the unique needs of each cancer survivor. Oncology nurses and advanced practice nurses will play a crucial role in the development of survivorship care from education and assessment to the delivery of coordinated care.

Evolving nursing science and practice in cancer survivorship.

To review the status of cancer survivorship care planning and delivery, resources and tools available to advance care, and explore professional nursing's potential to affect the quality of care available to cancer survivors. Published peer reviewed literature, web-based resources, and cancer-related professional organizations' resources. The population of cancer survivors is characterized as "booming," and available resources are also booming. Professionals involved in planning and delivering cancer survivorship care have access to tested tools, resources, information, and data useful for programmatic strategic planning and individualized survivor care plans. There are significant challenges to implementation, but there are also hopeful indicators that holistic care and services can be both cost-effective and used to improve care and quality of life for survivors. Nurses have significant roles in the planning and delivery of survivorship care. There are as yet no clearly defined pathways, guidelines, and standard metrics that reflect the value of these nursing roles, though there are interesting and intriguing early indicators of value that should stimulate the imagination of oncology nurses regardless of care setting, geographic location, and population to be served.
